Records packets, per provider and per case lane
Pick the lane, name the providers, and download a single PDF for each one: cover sheet, the HIPAA right of access request, the accounting of disclosures request, the preservation / litigation hold notice, and a service log page to date and file.
1. Case lane
The lane sets the cover sheet: forum, scope and the identifier line.
Custody, visitation and reunification. Records that show the actual history of care, contact and every report sent about our family.
Usual recipients in this lane: Treatment program that reported to the court · Hospital or clinic named in a report · County department that filed or supplied a report · School or childcare provider
2. What goes in
Every packet carries the preservation notice — a request without a hold lets a retention schedule answer first.
- Enclosure 1: Lifetime medical records — HIPAA right of access45 CFR § 164.524 · response clock: 30 days, one 30-day extension on written notice
- Enclosure 2: Accounting of disclosures — who saw my file and why45 CFR § 164.528 (and 45 CFR § 164.524 for the access log itself) · response clock: 60 days, one 30-day extension on written notice
- Enclosure 3: Litigation hold / preservation noticeDuty to preserve evidence relevant to pending and anticipated proceedings · response clock: Effective immediately on receipt
3. Providers
One PDF per provider, addressed to them by name — no bracketed placeholders left in the letter.
These are requests, not filings.
Every letter is Jake’s own request for his own records, electronically signed under E-SIGN and ESRA. Nothing here releases records to a third party, and anything filed with a court still carries his personal signature. Read the letters in full.
